REPLACEMENT CERTIFICATE/WALLET CARDS

The Great Lakes Regional OTI Education Center will be offering customers the opportunity to purchase replacement certificates and wallet cards online.

COST:

$10.00 per certificate or wallet card

CERTIFICATE TYPES:

Traditional Certificate: Replacement for a traditional 8.5" x 11" paper certificate suitable for framing on a wall.

Wallet Card: Replacement for a plastic wallet card that fits in your pocket, easy to carry!
Note: If you need a replacement OSHA 10/30 Hour Card, or Trainer Card, please ask your instructor to request one here.)
